Although learning from safety incidents is not routine outside our institutional boundaries, we have seen promising work in the many associations, networks and other groups that we belong to, including of course EBSA.
These groups are valued by members because they offer an environment of psychological safety and a culture of enquiry - essential for learning from incidents and improving safety.
We are organising a breakout session as part of the EBSA conference this year to explore how these existing networks can better support learning from incidents.
To help us prepare for this, we would be grateful if you could let us know about local networks or groups and how incident information about incidents is shared within them.
